Kindergarten - Grade 12 Registration

It's time to register your child for Kindergarten!

Per board policy, students must be age 5 on or before the first day of school the year in which he/she applies for entrance.
Announcements about kindergarten screening dates will be posted to the district website.

REQUIRED REGISTATION DOCUMENTS

The following documents are required for registration for K-12 within Final Forms:

  1. Proof of Residency – consisting of a deed, rental agreement (with all parties listed), gas or electric bill in parent/guardian’s name, property tax statement, voter registration card or completion of Form 5111 F2a and F2b.
  2. Birth Certificate (a certified copy from the Bureau of Vital Statistics)
  3. Custody papers (if applicable)
  4. Immunization Records
  5. Parent/Guardian Photo I.D.
  6. IEP/MFE (if applicable)
  7. Transcripts (Mandatory for Grades 10-12) or Current Report Card (Grades K-9)
  8. Dentist Report (Availalbe in Final Forms) (Kindergarten and Preschool only)
  9. Healthcare Provider Report (Availalbe in Final Forms) (Kindergarten and Preschool only)
  10. Kindergarten Parent Survey (Availalbe in Final Forms) (Kindergarten and Preschool only)
  11. OST (Ohio State Tests) Scores from Previous School (Grades 3-8)
  12. EOC (End of Course) Exam Scores from Previous School (Grades 9-12)
